In an effort to save time, we ask that prescribers include the following information on their prescriptions when writing an order for medical equipment.

 

  • TYPE OF EQUIPMENT BEING PRESCRIBED

  • DIAGNOSIS OR DIAGNOSIS CODE

  • LENGTH OF NEED

  • PATIENT WEIGHT & HEIGHT IF NON-STANDARD EQUIPMENT BEING ORDERED*

* If a patient's weight is over 250lbs., please include height & weight on prescription.

 

By including this information on the prescription, additional paperwork can be avoided.

 

 

Helpful tips for writing DME prescriptions:

 

Cane Prescriptions:  Please indicate whether you would like a standard or quad cane.

 

Walker Prescriptions: Use the photos to help with selection.  Please indicate if larger size is needed and note patient weight on prescription.

Wheelchair Prescriptions: Please indicate Lightweight, Standard, Wide or Extra-Wide.  Also include whether or not leg-rests are needed or not.  Please indicate patient weight on prescription if over 250lbs.

 

Hospital Bed Prescriptions: type of bed (example: semi-electric hospital bed w/siderails).  All of our in-stock hospital beds are standard size.  If a larger size is needed, please indicate on prescription and there will be a 2-4 day waiting period.  Please indicate length of need along with diagnosis on prescription.

 

Commodes: Standard 3-in-1 commode, Drop-arm commode, or bariatric commode.  Remember diagnosis and length of need on prescription.

 

Lift Chairs: Medicare patients must pay non-Medicare covered cost before chair will be ordered.  Medicaid will cover the cost of the entire chair with proper diagnosis.  Please indicate diagnosis and length of need on prescription.  (Note: The patient must be able to ambulate either with or without assistance after reaching upright position to qualify for chair.)

 

Patient Aids: Patient aids don't require a prescription, but we ask that you write one or at least write a note to help the patient remember exactly what they might need.  We carry toilet seat risers, grab bars, shower chairs, tub bars, etc.  If a particular item isn't in-stock, we can generally receive it the next business day and have it delivered to the patient.
